If you are a new client, please print, complete and return the following
forms prior to your initial consultation.
We ask that these forms be received in our office no later than two
business days prior to your first scheduled appointment.
We also encourage you to provide all previous testing, school records, or
other relevant information for our review.
We recommend that you mail the completed forms to 777 Forest Street, Reno, NV 89509. You may also scan an email the forms
in a PDF file to

. However, please note that because email is not a secure form of
communication, confidentiality cannot be guaranteed.
